New Patients
Starting Your Wellsmart Journey
I'm a new patient. How do I get started with Wellsmart?

Wellsmart is a program-based practice. All new patients enter through a simple, structured pathway. There are no separate intake fees or deposits — your program enrollment is your commitment, and it is what secures your first visit with Dr. Coleman.

New Patient Pathway

Step 1 — Watch the introduction: Begin by watching Dr. Coleman's brief introduction to the Wellsmart Care Program, available on our website. This ensures you arrive at your discovery call with a clear picture of what the program involves.

Step 2 — Discovery call with Kelsey: A 20-minute call with our Care Manager, Kelsey Dalton. She will learn about your health situation, describe the program in detail, and together you will determine whether this is the right fit. If it is not, we will tell you honestly — and help you find a more appropriate provider if possible.

Step 3 — Enroll and book your first visit: If fit is confirmed, you will receive a program enrollment link. Annual or monthly payment at time of booking secures your first visit with Dr. Coleman. No separate deposit required.

Step 4 — Your Wellsmart year begins: Your first visit is a 90-minute consultation with Dr. Coleman — the first clinical act of your committed program year. Labs, protocols, and recommendations follow from within the program.

This structure exists because functional medicine requires a committed care relationship to work properly. Labs ordered without that framework have nowhere meaningful to go. Everything Dr. Coleman learns in your first visit has a program to live inside.

Which program tier is right for a new patient?

For all new patients, the Wellsmart Care Program is the starting point. Functional medicine during the first year involves active pattern recognition, lab interpretation, protocol adjustments, and consistent physician engagement. The Care Program includes three physician consultations per year, a structured annual care arc, and priority communication — the clinical infrastructure that new patients genuinely need.

Wellsmart Essential is available to established patients who have already completed at least one full program year and are maintaining their health with lighter-touch ongoing support. It is not offered as a starting point for new patients.

Wellsmart Restore is available by invitation only for patients committed to a defined 12-month transformation arc with maximum physician involvement.
Why are labs ordered after program enrollment rather than at the first visit?

Because labs are most meaningful within a longitudinal care relationship — not as a standalone event.

Comprehensive functional medicine lab panels identify patterns that Dr. Coleman will track and interpret over time, adjusting protocols across multiple visits as your system responds. Without the program framework in place, there is no structured follow-up, no longitudinal tracking, and no clinical context in which to act on results meaningfully.

Your first visit is the beginning of a program year that is already committed and in place. Everything Dr. Coleman orders and recommends has a care relationship behind it.

In cases where Dr. Coleman identifies a clinical urgency during your first visit that requires immediate laboratory investigation, labs may be ordered at physician discretion regardless of timing. Patient care always comes first.
The Membership
What It Is — and What It Isn't
What exactly is a Wellsmart membership?

A Wellsmart membership is a 12-month physician-led care relationship — not a subscription to services, and not a payment plan for visits. It is the structural foundation that allows Dr. Coleman to practice functional and integrative medicine the way it actually works: continuously, longitudinally, and in response to how your system changes over time.

What membership provides is continuity. Your history is carried forward. Your patterns are tracked. Your Care Manager stays in contact. And your physician is not starting from scratch every time you need something.

It is also an educational partnership. Wellsmart courses, workshops, and monthly group sessions are designed to help you understand your body well enough to work with it — not just receive care passively.

Is this insurance? Does it replace my health insurance?

No. Wellsmart membership is not insurance, does not function as insurance, and does not replace your existing health coverage. This is a direct-pay, non-insurance-based membership for functional and integrative medicine services.

We do not bill insurance for membership services. Members are responsible for membership fees directly. You may continue to use your health insurance for services outside the scope of Wellsmart — such as specialist referrals, imaging, emergency care, or hospitalization.

Membership fees may not be reimbursable by insurance. Please consult your plan for details regarding HSA or FSA eligibility for direct-pay medical services.
What is not included in the membership?

Membership covers the care relationship, educational access, group sessions, Care Manager support, and tier-specific physician consultations. It does not include:

· Laboratory testing or diagnostics (billed separately at applicable rates)
· Medications or supplements
· Imaging services
· Emergency or urgent care outside Wellsmart's scope
· Hospitalization or specialist referral fees
· Disability evaluations, workers' compensation, or administrative/legal documentation

Physician consultations beyond what is included in your tier are available at discounted member rates ($275 per visit vs. $315 standard).

The Tiers
Choosing the Right Level of Support
What is the difference between Essential, Care, and Restore?

Wellsmart Essential ($2,149/year or $229/mo) provides the foundational care relationship — Care Manager support, monthly group sessions, educational access, and discounted physician consultations. Physician visits are not included but available at member pricing. Best suited for established patients maintaining their health.

Wellsmart Care ($3,780/year or $402/mo) adds three included physician consultations per year, a structured annual care arc, priority communication, and one Coherence Training session. This is the recommended tier for patients with active health conditions or those new to functional medicine.

Wellsmart Restore ($6,426/year or $685/mo) is available by invitation only. It includes quarterly physician reviews, two Coherence Training sessions, personalized protocol design, dedicated annual health arc documentation, and same-week response priority. Designed for patients undertaking a defined 12-month transformation arc.

Can I upgrade my tier during my membership year?

Yes. If you find that your health needs require a higher level of support during your membership year, upgrading is possible. Upgrades are handled through Kelsey and take effect at the start of the following billing cycle. You would move to the then-current rate for the higher tier.

If you are uncertain between tiers at enrollment, it is worth having a conversation with Kelsey before you commit. Choosing the right tier at the start is simpler than upgrading mid-year.

Can family members join my membership?

Yes. Additional household members may be added to any Wellsmart membership for $75 per person, per month, billed as part of your primary member's 12-month term. Family add-on members receive access to monthly group sessions, all Wellsmart educational programs and courses, and physician consultations at member pricing.

Family add-on members share the primary membership's care infrastructure but do not receive their own included physician consultations unless enrolled in a tier that provides them. If a family member has active health needs requiring regular physician involvement, a separate membership may be more appropriate.

Payments and Commitments
Understanding the Financial Structure
Why is this a 12-month commitment rather than month-to-month?

Because meaningful functional medicine requires time. The patterns Dr. Coleman tracks — metabolic, hormonal, immune, nervous system — unfold across months and seasons, not weeks. A 12-month commitment creates the continuity that allows us to actually see what's changing and respond to it with precision.

Month-to-month membership would undermine the clinical model. It would create uncertainty in the care relationship and make it difficult to plan your care arc, order sequential labs, and track longitudinal changes. The 12-month structure is not administrative — it is clinical.

What happens if I need to cancel before the year is up?

Early termination is possible with 30 days written notice, but it triggers a financial reconciliation process. The practice will calculate the fair market value of all services rendered at standard non-member rates and compare that to the total payments made. If services exceed payments, the balance is due. If payments exceed services, the difference is refunded.

This process exists because membership pricing reflects a year-long commitment. Services delivered early in a membership year may be valued at more than the monthly installments paid to that point.

Ongoing Care
What Membership Looks Like Day to Day
What are the monthly group sessions, and do I have to attend?

Monthly group sessions are virtual Q&A sessions and specialty subject matter group visits, led by Dr. Coleman — open to all members across all tiers. Each session is an opportunity to ask questions, hear Dr. Coleman's clinical perspective on topics that are coming up across the practice, and connect with other patients who are navigating similar health journeys.

Attendance is not required, but it is encouraged. These sessions often surface insights and answers that benefit members beyond what a one-on-one visit can provide. All sessions are recorded and available to members who cannot attend live.

What if I have an urgent health concern between scheduled visits?

For urgent or emergent medical issues — chest pain, difficulty breathing, acute injury, or anything that feels like a medical emergency — please call 911 or go to your nearest emergency room. Wellsmart is not an urgent care or emergency practice.

For urgent-but-not-emergent concerns — a new symptom that worries you, a question about a medication, an unexpected lab result — your Care Manager is your first point of contact. Kelsey is equipped to triage your concern, provide guidance within the scope of her role, and escalate to Dr. Coleman when physician input is needed. Response times vary by tier, with Care and Restore members receiving priority communication.

What membership provides is the assurance that you are not navigating these moments alone.

Are physician consultations virtual or in-person?

As Wellsmart operates as a virtual-first practice, the majority of physician consultations are conducted via telehealth — secure, HIPAA-compliant video visits. This removes geography as a barrier to your care and allows for meaningful clinical conversations from wherever you are.

In-person visits may be available on a limited basis depending on scheduling. Please ask Kelsey about current in-person availability when you enroll.
